Labiaplasty is a surgical procedure that reduces the size of the labia minora (the inner lips of the vagina) and improves the overall appearance of the vagina. For some women, an enlarged labia minor causes discomfort while playing sports, wearing tight clothing, and even during sex. Reducing the size of their labia minora goes a long way to relieve these issues with little risk or downtime.

WHO’S IT FOR?

Some women are born with enlarged labia, while others may develop the enlargement with age or after childbirth. Many other women simply want to improve the appearance of their genitals.

Medical conditions that may contribute to abnormalities of the labia are:

  • Trauma/injury to the labia
  • Tearing of the labia minora during childbirth
  • Stretching of the labia

THE PROCEDURE

Performed in operating rooms or occasionally by some doctors in an exam room, the labiaplasty procedure can take up to several hours to perform. Local or general anesthesia is administered and is dependent on whether other alterations are being made during the course of the labiaplasty.

There are several different techniques that can be used in labiaplasty. The two most common are the Trim Technique and the Central Wedge Technique.

The Trim Technique is the original labia reconstruction technique. It effectively consists of a trimming of the excess tissue and results in a scar line replacing the labial edge. The scar can lead to chronic discomfort and an unnatural look.

The Central Wedge Technique was invented 20 years ago by Dr. Alter who was eager to provide women with a solution that preserved the normal contour, color, and anatomy of the labia minora edge. With his innovative technique, a wedge of tissue is removed from the thickest part of the excess labia and then stitched back together. This allows for a more natural contour than the trim technique.

WHAT’S THE RECOVERY LIKE

Surprisingly, the recovery time associated with a wedge labiaplasty is quite minimal as far as surgical procedures go. In most cases, patients can return to work five days after surgery and sexual activities can be resumed in about six weeks. Patients are asked to avoid any strenuous activity for some time after surgery.

WHAT RESULTS CAN BE EXPECTED

A 2014 study found that more than 91% of women who had the surgery felt more satisfied with their genital appearance afterward.1 Some patients have noted marked improvement in function and less friction and irritation during physical and sexual activities. Many other patients have noted an increase in sexual satisfaction.

THE SIDE EFFECTS

It’s normal for patients to experience soreness, bruising, and swelling in the area for up to two weeks post-op. No loss of sensation or decline in orgasmic ability have been associated with this procedure once site has healed completely.

Women are becoming more open about their sexual wellness. and that’s a really good thing. According to a recent study done on the prevalence of female sexual dysfunction “over 40% of women have psychological distress from female sexual dysfunction, and only 14% consult a physician about it.”1 That’s over a quarter of women who are missing out on the many benefits of a healthy sex life. That’s over a quarter of women suffering in silence. I’ll let that sink in for a moment.

For too long, female pleasure and the parts of their bodies’ that enable such pleasure has long been regarded as taboo and firmly avoided by the mainstream. Often regarded as improper or scandalous or even worse, this long-held falsehood prevented many women for seeking out assistance when their own biology prevented them from experiencing sexual fulfillment.

In comparison, erectile dysfunction, the male equivalent of sexual dysfunction in women, has been studied at great length and a plethora of options exist for treating it. Studies for women are picking up some steam, but to this day primary mainstream treatment is limited to psychological treatment, hardly a big leap from Freud’s clinical days.

FEMALE SEXUAL DYSFUNCTION: WHAT’S THE BIG DEAL?

It’s actually a really big deal, according to recent studies on the effects sex has on overall health and wellbeing. Several studies have linked cardiovascular diseases with sexual dysfunction in females as well as males. According to a study done in 2008, they found that 87% of middle-aged women with heart failure reported some degree of sexual dysfunction. Of these patients, “80% reported reduced lubrication that resulted in frequent unsuccessful intercourse (76%) and 63% of the same population had difficulties achieving an orgasm.” These alarmingly high numbers suggest that sexual dysfunction most certainly has a huge effect on the quality of life for women—both physically and psychologically—and could even be an early indicator of a life-threatening/fatal disease.

It’s estimated that roughly a thousand women a year experience a botched labiaplasty, and those numbers are only expected to climb as the rate of labiaplasties performed worldwide continue to rise. The leading causes of botched labiaplasties come down to surgical error and lack of specified surgical skills on behalf of the surgeon. Sometimes it’s the case of a difficult anatomy and other times it’s due to the patient not following post-op care. 

What Exactly is a Botched Labiaplasty?

When a labiaplasty goes wrong, women are left with less than ideal results. For Dr. Alter, nearly 40% of all labiaplasty he has performed have been reconstructive labiaplasties as a result of a previously botched labiaplasty. That’s a daunting number to be sure.  

Typically, in a botched labiaplasty the surgeon has unskillfully removed the extra tissue causing either painful scarring or no sensation at all. Often, they remove too much labia resulting in a total amputation with extra hood tissue hanging from the top that leaves women feeling mutilated. 

Unfortunately, when surgeons remove too much labia tissue it can be very difficult for more experienced surgeons, like Dr. Alter, to repair the damage done. That’s why it is extremely important to research perspective surgeons before trusting them with your labiaplasty. The time to get a labiaplasty as perfect as possible is in the first go. Financial reasons should not overshadow the importance of expertise and hands-on experience.

When is Revision or Reconstruction on Option?

If you are experiencing pain, discomfort, lack of sensation, struggling to orgasm, and are overall displeased with the aesthetic outcome of your previous labiaplasty then you should consider contacting an experienced surgeon like Dr. Alter with a long history of revisiting and repairing botched labiaplasties.

Ideally, the revision required post-original labia reduction would be minimal, and you may consider using your previous surgeon for the touchups. But sometimes that’s not the right decision and a new perspective and expertise is warranted. In that case, it’s imperative that you find a surgeon who has the skill to do the job. In his experience, Dr. Alter has found that the typical gynecologist does not have the specific training in cosmetic techniques necessary to do labiaplasties in a way that delivers optimal aesthetic outcomes.

How Long Should I Wait After a Botched Labiaplasty?

Dr. Alter advises anyone who has just undergone a labiaplasty to relax and chill for the first six weeks. Easier said than done, but this part is crucial. Because post-labiaplasty your genitals are not going to look or feel fantastic for a while. You need to give your body time to heal and stressing out about the results so early in the healing process will only do more harm than good.

After about six weeks, you should have a better idea of whether your labiaplasty was a success or not. If you are unfortunate to have experienced a botched labiaplasty, Dr. Alter and any well-skilled plastic surgeon with specific training in vulvovaginal aesthetics will tell you that no revision or reconstruction surgery should be attempted until ~4-6 months post-botched labiaplasty. Everybody heals at different rates and it’s important that no attempts to fix a botched labiaplasty takes place until everything is healed. You really only have one more chance to get it right and you want to make sure you get it right this time around. 

Preventing a Botched Labiaplasty Is Always Your Best Bet

Dr. Alter has heard it time and time before. Patients go into their labiaplasty assuming that the surgeon who is performing their labia reduction has the knowledge and expertise to pull off a flawless labiaplasty. They went to medical school after all and are well-trained, respectful M.D.s. But like most professions, not all surgeons are qualified to perform labiaplasties. 

Other women base their decision on whomever offers the cheapest option, but like the saying goes you get what you pay for. And entrusting your labiaplasty to the cheapest surgeon will almost always end up costing you way more in the end. Remember, your vaginal health and overall wellbeing are at stake here.

The best way to avoid a botched labiaplasty is to do you due diligence and find a surgeon who has done many labiaplasties and has a very high success rate. Look for a surgeon who is well versed in the various techniques available to labiaplasty surgeons. And, most importantly, ask questions. Know precisely what will be done before it’s done and how much they will take off and where. Educate yourself on the anatomy of your vagina and what the possible side effects might be. Even the greatest labiaplasty surgeon can make a mistake so it’s best that you know precisely what you are getting into so you can weigh your options before it’s too late. Like the other adage goes, once something is removed it’s harder than heck to get it back.

So you’ve decided to get a labiaplasty, or looking to fix a botched labiaplasty. Great on you. But getting the procedure is just a faction of the journey. Knowing how to care for your incisions and yourself after your labiaplasty will ensure your recovery is smoother so you get the results you deserve.

Just like any surgery that requires a recovery period, it’s really important to stock up and mentally prepare yourself for the next few weeks post-operation. Here are a few dos and don’ts that will help you heal effectively and possibly make your road to recovery as smooth as possible.

First with the Dos:

Relax and take it easy. It’s important to go easy after any surgery, and labiaplasty definitely falls under that umbrella. Your body will be busy trying to heal the incisions, so make sure to rest and give your body time to heal. It’s best to lay down on your back and avoid sitting upright. But it’s also important to walk around a bit to maintain healthy circulation and avoid blood clots. Just don’t overdo it the first month.

Apply a cold compress. Icing the affected area right after surgery will go a long way to reducing inflammation and discomfort if done routinely and regularly. But don’t leave the cold compress on for more than 15 minutes at a time to avoid damaging the skin.

Take prescribed medication. Be sure to follow post-op instructions, including taking prescribed medication. You may be prescribed an ointment as well. Alcohol use should be avoided while taking prescribed pain medication. Check with your doctor before taking non-prescribed or OTC medications to be safe.

Check your incisions daily. Infections can set in even under the best of care. So be sure to check the site daily for possible infection. If you see any indication of an infection, call your doctor’s office immediately for next steps.

Keep the area clean and dry. Your incisions will not heal properly if the area isn’t kept dry and clean. So make sure to toss out bandages once soiled and replace with a sanitary napkin which should be replaced regularly. Your surgeon will instruct you on how to do this and you should follow all instructions carefully.

Eat a balanced diet. Look you’re probably wondering about bowel movements and urinating after surgery because who wouldn’t! To minimize, ahem, complications it’s advised that you eat a high-fiber diet rich in fruits and vegetables. You should be doing that any ways, but really be sure to do it post-labiaplasty. The high-fiber will stave off constipation which is crummy when all is well down south. Oh, and drink loads of water. Hydration is crucial to healing and it will ensure your urine isn’t abundantly acidic which will go a long way to eliminate painful stinging during urination.

Speaking of urination, use a spray bottle filled with water. Stinging during urination is pretty common post-labiaplasty just like after childbirth. Keep a spray bottle with water close by and squirt it gently at your vagina when you pee. This will dilute your urine, minimizing the burning sensation and keep you clean in the process. 

Call your doctor’s office immediately if you experience:

  • Significant bleeding from the surgical site.
  • A fever over 100.4° F within the first three days.
  • Significant pain at surgical site that doesn’t subside with pain relievers.
  • Excessive drainage or redness around the surgical site.

Now for the Don’ts:

Stress and freak out. Seriously. You just had surgery on your lady bits. They are going to be swollen, sore, and itchy. This is to be expected. So don’t constantly look at yourself in the mirror. Fixating on your parts the first few weeks after surgery will only stress you out further and possibly inhibit healing. Anxiety has been shown to slow down the healing process. So, seriously, relax and be gentle on yourself.

Have SEX. Best to stock up on the sex prior to surgery because it’s going to be at least six weeks before you can have intercourse again. Don’t mess around with your lady parts. Let them heal completely post-labiaplasty and don’t rush the sex. And once you get the go ahead from your surgeon, go easy and listen to your body. 

Lift heavy stuff or do strenuous activity. It’s important to go easy the first month post-labiaplasty or even longer depending on your surgeon’s instructions. That means no lifting weights or heavy objects, running, horseback riding, bicycling, walking long distances, or even sitting for prolonged periods. A good rule of thumb: If it hurts your mending vagina in any way, don’t do it.

Soak in a bath or go swimming. It’s important to avoid soaking your vagina until the incisions and scar have properly healed. Soaking in hot water will only elongate or, worse, complicate the healing process and should be avoided entirely.

Smoke or use drugs AT ALL. This includes everything. Smoking and other drug use can slow down the body’s ability to heal itself, prolonging and even complicating the healing process needlessly. So be sure to eliminate these prior to surgery and absolutely stay away from them while you heal. Or, take this as the perfect time to quit them once and for all.

On top of all these Dos and Don’ts, it’s incredibly important to maintain a positive outlook on life as you start your healing journey. Use this time to open that book you’ve been dying to read, or binge your favorite shows. Invite friends over to keep you company and make the most of your recovery. And if you suspect any issues post-surgery, do not hesitate to notify your doctor’s office. It’s better to be safe than sorry.
